2. Provide feedback on what is happening so the top           management can shape appropriate actions to               respond  to changing circumstances

The company need to develop a procedure on how to conduct incident investigation. 
Incident investigation is a process for reporting, tracking, and investigating incidents to identify the failures in OSHMS and should be documented.
