The  physician 
1-Prescribes the medication order 
2-Document it on EMR(Electronic Medical Record)
3-should review the patient allergy 
Medication order contain:
The 5 rights which are 
            -Medication

-Dose and regimen

-Route of administration

-Time
 
             -Frequency of administration

Clinical pharmacy 
1-Reviews the reason/indication of medication therapy
2-Distribute the medication time on EMR
3-Verify the medication order

THE Qualified Nurse
1-Document the administered medication  in EMR
2-Double independent check of the 5 rights
 
signing of a witness (second qualified nurse).
 STAT Orders
(within 45 min
from order time).

Nurse role is to Ensure
STEP

In case of the patient is not given his medication or he  refused
the medication is NOT left beside the patient
inform the  physician
The Nurse
 record it 
follow his instruction 
return the medication to the clinical pharmacist

Regarding the administration of Psychotropics, controlled, and narcotic medications
1-The previous processes are followed
 with Documentation
2-Narcotic prescription pads including the signature of healthcare provider.

   A-Clinical pharmacists Role
1-Educate the patient about  the medication and potential adverse drug reaction 
2-Give him chance to ask questions to understand.
  B- Responsible nurse

Educates the patient and-his family about medication route,duration, frequency.
