Bring a list of issues to discuss with your provider
Try to keep it brief
so there will be enough time to address your most important concerns.
Just don't write like this!
Doctors, nurses, and your medical team love to care
for you and always want to do their best! 
But unfortunately...
time is limited
So keep your discussions focused on
